Care Home Assistant Practitioner (CHAP’s)

As a Care Home Assistant Practitioner (CHAP) at Lakeview Care Home, you will play a crucial role in bridging the gap between the responsibilities of a care worker and a nurse. CHAPs are integral members of our team, getting involved in both the management and clinical aspects of residents’ care. You will carry out a variety of tasks, many of which are similar to those undertaken by nurses in a care home setting.

Key Responsibilities:
– Assist in the development and implementation of care plans for residents.
– Administer medications under the supervision of a registered nurse.
– Monitor residents’ health and well-being, reporting any changes or concerns to the appropriate medical personnel.
– Provide personal care to residents, including assistance with bathing, dressing, and grooming.
– Support residents with activities of daily living, encouraging independence wherever possible.
– Assist with the coordination of activities and events for residents, promoting social engagement and stimulation.
– Maintain accurate and up-to-date records of residents’ care and progress.

Requirements:
– Minimum of two years’ experience as a senior care assistant working with service users living with dementia.
– NVQ Level 3 in Health and Social Care or equivalent qualification.
– Excellent communication and interpersonal skills.
– Ability to work effectively as part of a multidisciplinary team.
– Compassionate and empathetic approach to resident care.
– Strong organisational skills and attention to detail.
– Ability to remain calm under pressure and handle challenging situations with professionalism.
